WellPath 2022-2023 Contract Campaign – BARGAINING TEAM NOMINATION FORM

Nominations for seats on the 2022-2023 SEIU 721 WellPath Bargaining Team are now being sought.

Nominations
• You may nominate yourself by completing the Nomination Form below.
• This is not a ballot. Only one nomination form per candidate is necessary. Submitting more than one nomination form will not increase your chance of winning a seat.
• Submit your nomination form by Friday, December 16 at 5PM  (PST).

Elections
An election will be conducted if the number of candidates exceeds the number of available seats on the Bargaining Team. There are 5 bargaining seats available.

Only SEIU 721 WellPath members in good standing may submit a valid bargaining team nomination.
