LA County: $1,000 SEIU 721 bonus will be in Nov. 30 paychecks

Nov. 24 is the last day for SEIU 721 Members to apply for additional bonuses!

Employees who made mistakes on Self-Attest Forms can make corrections online in January

All of us who are LA County employees and represented by our union, SEIU 721, should get a $1,000 bonus in the Nov. 30 paycheck.

But if we worked at “elevated risk” during the pandemic, there are additional bonuses we qualify for.

(“Elevated risk” = Job duties required employee to be within six feet of a co-worker, patient, client or member of the general public for a period of at least 15 minutes on at least one occasion during the pandemic.)

If we work at the Department of Health Services, there are TWO additional bonuses we may qualify for!

All we have to do is submit a Self-Attest Form. But be sure to do it soon. Wed., Nov. 24 is the LAST DAY to submit it!

Then click on ‘eFORMS’.

The forms will be found under the “Countywide” tile. After completion of the $500 Countywide “COVID Appreciation” bonus form, DHS employees will be taken to another form to request the DHS Heroes Bonuses.

 

BONUS DETAILS:

$500 Countywide COVID Appreciation Bonus: To qualify, we must be full-time employees (or part-time employees who worked an average of at least 20 hours per week during the pandemic); who were employed from July 1, 2020 through June 30, 2021; and who worked at elevated risk. “Elevated risk” is defined as a job duty that required us to be within six feet of a co-worker, patient, client or member of the general public for a period of at least 15 minutes on at least one occasion during the pandemic.

$150 DHS Bonus: To qualify, we must be permanent, full-time employees who were employed by the Department of Health Services from March 1, 2020 through June 30, 2021 and who remained employed by DHS as of November 2, 2021.

$500 DHS “Heroes” Bonus: To qualify, we must be permanent, full-time employees who worked at “elevated risk;” who were employed by the Department of Health Services from March 1, 2020 through June 30, 2021; and who remained employed by DHS as of November 2, 2021. “Elevated risk” is defined as a job duty that required us to be within six feet of a co-worker, patient or member of the general public for a period of at least 15 minutes on at least one occasion during the pandemic.

Only DHS employees can qualify for the $150 DHS Bonus and the $500 DHS “Heroes” Bonus. That’s because those dollars are paid out of Provider Relief Fund moneys received by the Department of Health Services in the original federal CARES Act. It does not allow for expenditures outside of DHS.

But if you qualify – it’s time to apply!

ALERT: Those of us who made errors on our Self-Attest Forms will have an opportunity to make corrections online from January 1 – January 15. So, if you made a mistake, don’t despair!

These bonuses are all made possible thanks to the power of our union. We, the members of SEIU 721, elected strong bargaining teams to represent us … and the results speak for themselves.

Is this only for LA County Employees or will City of LA employees that are part of SEIU721 be getting this as well?

If you fill out those forms after November 24th , because you forgot or you were on vacation or any other reasons , will you also be able to fill out those forms in January of 2022 ? And I’m guessing you’ll receive you money at a later date?

Hello all, So I have been an RN working at LAC+USC since 3/1/2021. Does this mean I do not qualify? No worries if not, I just want to make sure as no-one can seam to tell me for sure. Thank you for all you’ve done for us!

What is the start date for “during the pandemic”? If I worked in office between March 1st, 2020 and March 15th, 2020, do I qualify for the $500 Countywide COVID Appreciation Bonus?
