If you haven’t already received materials in the mail notifying you to update your life insurance Beneficiary Form – you should have. So be sure to check your mailbox! Here’s a sample of what the new form looks like:
REMEMBER: Because you are an active SEIU 721 member, you ALREADY enjoy this life insurance benefit. This is NOT a new offer and there is NO additional cost.
Active SEIU 721 members are automatically covered for $2,000 life insurance and an additional $2,000 for accidental death and dismemberment insurance (a total of $4,000 if accidental death). But you MUST update your Beneficiary Form NOW – because the information you provide Zenith American Solutions today will supersede any previous Beneficiary Forms that Local 721 has on file.
SEIU 721 recently hired Zenith American Solutions – an experienced administrator – to handle beneficiary benefits in order to ensure the highest quality of service to all our members. To guarantee that your beneficiary information is accurate and up-to-date, we are asking all SEIU 721 members to provide Zenith with a new, completed Beneficiary Form.
After filling out the new form, choose one of these options
- Email it to SEIU721Beneficiary@zenith-american.com
- Fax it to (415) 227-0552
- Return it in the mail using the return envelope provided (the return address is 1325 North Grand Avenue, Suite 200, Covina, CA 91724)
Please DO NOT send your updated Beneficiary Form to SEIU 721 or to Trustmark Life Insurance. Be sure to send it to Zenith American Solutions by fax, email or mail using the information provided above.
