By now, you should have received materials in the mail notifying you to update your life insurance Beneficiary Form. Here’s a sample of what the form looks like.
After completing the form that you receive in the mail, choose one of these options:
- Email it to SEIU721Beneficiary@zenith-american.com
- Return it in the mail using the return envelope provided
- Fax it to (415) 227-0552
Please DO NOT send your updated Beneficiary Form to SEIU 721 or to Trustmark Life Insurance. Be sure to send it to Zenith American Solutions by fax, email or mail using the information provided above.
Remember: Active SEIU 721 members are automatically covered for $2,000 life insurance and an additional $2,000 for accidental death and dismemberment insurance (a total of $4,000 if accidental death). The information you provide Zenith today will supersede any previous Beneficiary Forms that Local 721 has on file.

To guarantee that your beneficiary information is accurate and up-to-date, we are asking all SEIU 721 members to provide Zenith with a new, completed beneficiary form.
